One of our Physician Contracting Representatives will provide, among other documents, a credentialing application during their first office visit. You need to complete the credentialing application and provide all documentation specified in the checklist, through e-mail at <a href=\"mailto:prcredentials@humana.com\">prcredentials@humana.com<\/a> or fax at 1-866-423-6635. The checklists are identified below by area of service.
